HOW TO GET A SPONSOR FORM

What does it mean to be a Sponsor for Baptism or Confirmation? To put it in contemporary terms, it means to be a mentor in the ways of faith. As a result sponsors are required to be Catholics in good standing so that they can properly fulfill their role of mentoring someone in the the Catholic Faith. According to Cannon Law this means that they are going to Mass on a regular basis, have received all the sacraments of initiation (Baptism, Confirmation, and Communion), and married in the Church if married. Only one sponsor is needed for baptism, but if there are two they can not be of the same sex. It is also possible to have a non-Catholic Christian be a Christian Witness.

Sponsors are required to get a form from the parish to which they presently belong to show that they are qualified to fulfill this role of being a mentor for the Catholic faith.

To get a sponsor form from one of our two Parishes you first need to register in the parish using Flocknote. The parishes do not have a parish database, thus we are asking even long-term parishioners to re-register using Flocknote. The link below will bring you to the web page with instruction on how to register with Flocknote. Please note that there will be one question titled “Type of Registration”. Click on it and select “Sponsor form”.

After you have registered in Flocknote call the office to find out what Mass Fr. Babiczuk is celebrating. You can leave a message for Shanna, and she will either call you back or text you with the information. You can pick up the sponsor form from him before or after Mass.

If you have already register in Flocknote then simply call the office as described above.
